Initial Session Information and PaperworkActive Minds Counseling & Wellness is dedicated to meeting the personal and/or professional needs of every individual we create a relationship with. That being said it is important that in establishing our professional relationship there are terms that we must agree to. Prior to your initial consultation, please read and sign the Client Policies/Authorization for Consent/HIPPA Form and Adult or Child/Adolescent Intake Form and bring both forms to your initial consultation.
Cancellation Policy- There is a full session fee for any appointments that are cancelled with less than 24 hour notice.
Payment- Payment is made prior to the session. Cash, credit card (Visa, Mastercard, American Express) and some insurances are accepted.
Reduced Fee Services- A set number of sliding fee slots are available per month. If you think you may qualify, please inquire.
